Stanford University, the landmark study
In 2024, Stanford published the first controlled clinical study of the root’s primary alkaloid: thirty U.S. Special Operations veterans, one session each. One month on, PTSD, depression and anxiety had fallen by the figures above, disability scores went from 30.2 to 5.1, the level of no disability, and suicidal thinking fell from 47% of the group to 7%. Concentration, memory and processing improved. There were no heart problems. A 2026 follow-up with MRI found their brains measured younger after the session than before.

Unregulated,which is the whole story.
Mexico has not scheduled ibogaine, and COFEPRIS, its health regulator, neither prohibits it nor approves it as a medicine. That gap is what lets clinics operate, and it is also why no national standard governs them: each sets its own medical protocol, and quality varies from one to the next. The country holds the largest concentration of ibogaine clinics in the world, clustered on the Baja coast (Tijuana, Rosarito, Ensenada) and the Caribbean coast (Cancún, Playa del Carmen, Cozumel).

Unregulated, not approved
Ibogaine is absent from Mexico's controlled-substance schedules. COFEPRIS has not approved it as a pharmaceutical, so clinics may administer it under a physician but may not market it as an approved therapy.

No national clinical standard
Because the substance is unregulated rather than approved, clinics are not held to the standards of a hospital; the cardiac screening, monitoring and staffing on offer differ from clinic to clinic.

Texas chose a US trial over Mexico
Texas's $50 million IMPACT program funds FDA-track ibogaine trials at UTHealth Houston and UTMB, the first state program of its kind, a sign of where the research is moving.

Three of the clinics most often searched from the United States, with their location, program length and price as published on their own listings, checked October 12, 2026. Each is a medical program built around the isolated alkaloid; the ceremony on Kauaʻi is a different thing, and this page exists so the difference is plain before you choose.

A whole root ceremony, held in nature on Kauaʻi.
Most providers hand you a single isolated compound in a medical room. We work with the whole root, the way it has been held for millennia, in a sanctuary on Kauaʻi’s North Shore where the land itself holds you. Private journeys and containers of up to three members.
The root is a powerful and intelligent plant ally, and the science is catching up to it: in Stanford’s 2024 study, a single session eased veterans’ anxiety and depression and sharpened their thinking. Leaders and creators come for the same reasons: clarity, a sharper mind, open creativity, and performance that holds.

Thirteen weeks.Two nights in ceremony.
Six weeks of preparation from home. One week on Kauaʻi, with two held nights with the root. Six weeks of integration back home, so what the root shows you becomes how you live, lead and create.
Preparation
A personal medical consultation and health review with our medical provider. Somatic and nervous-system preparation. Weekly guidance through the member portal, so you arrive clear, steady and ready to receive.
Ceremony
An EKG on-island first. Then two fully held nights with the root, one-on-one or in a container of up to three, with experienced guides and our medical provider present overnight. Days of on-island integration between and after.
Integration
Six weekly one-on-one calls with your integration coach, plus somatic sessions, so the opening holds in your work, your relationships and your body.
Held by people who havewalked this road themselves.

What people askwhen they are comparing.
Is ibogaine legal in Mexico?
Ibogaine is not a controlled substance in Mexico, and COFEPRIS, the health regulator, neither prohibits it nor approves it as a medicine. Clinics operate in that gap, under a physician, with no national standard governing their protocols. In the United States, iboga and ibogaine remain Schedule I under federal law; Vital Kauaʻi holds the whole root in ceremony on Kauaʻi as a legally established church, within a protected religious context.
How is a whole root ceremony on Kauaʻi different from a clinic in Mexico?
A clinic administers an isolated, concentrated alkaloid in a medical facility over a week or so. We work with the whole root, titrated slowly over two held nights in nature, inside a thirteen-week journey: six weeks of preparation from home, one week on the island, and six weeks of one-on-one integration after. The full plant carries an intelligence that no single compound can replicate.
What should I ask any provider, in Mexico or anywhere?
Who takes my EKG, and when? Who is in the room overnight? Is it the whole root or an isolated compound? What happens the first week after I fly home, and who holds that? The answers tell you most of what you need to know.
Do I need a passport to come to Kauaʻi?
Members from the United States fly domestically to Līhuʻe (LIH), with no passport or border crossing. Members from abroad enter the US as they would for any visit; Hawaiʻi is domestic from there.
Is it safe?
The root has an excellent safety record. The rare serious risks are primarily cardiac, and are most closely associated with the isolated and concentrated alkaloid. We seek to meet the clinical standard of care wherever we can: a medical consultation and health review with our medical provider and medical clearance before ceremony, an EKG on-island before your first ceremony, and our medical provider present overnight at every ceremony.
Who is this for?
Leaders, creators, and people already on a spiritual path who are ready to live their fullest expression, with more clarity, freedom, capacity, and purpose. We screen carefully and honestly. If it is right for you, we will know together.
